About the role
The Role
As a Registered Nurse, the post holder will provide clinical leadership and direct nursing care to Service Users in their own homes. The role is central to meeting CQC requirements for the regulated activity of Treatment of Disease, Disorder and Injury, working closely with the Registered Manager to ensure strong clinical governance, oversight and quality assurance.
The post holder will ensure that all nursing interventions are safe, effective, evidence-based, and compliant with NMC standards, CQC regulations and Westwood Homecare policies.
This is a community-based role with no on-call requirement.
Key Responsibilities
Clinical Practice & Care Delivery
Deliver high-quality nursing care to Service Users in their own homes
Assess, plan, implement and evaluate nursing care for individuals with complex health needs, long-term conditions, acute illness or injury
Provide and oversee treatments including:
Wound care and pressure area management
Catheter care
PEG and enteral feeding support
Medication administration and clinical monitoring
Diabetes management
Infection prevention and control
Ensure all care is evidence-based and aligned with NICE guidance
Treatment of Disease, Disorder & Injury
Provide clinical oversight of nursing interventions within the scope of TDDI
Ensure safe delegation of nursing tasks to trained and competent staff
Monitor clinical outcomes and escalate concerns to GPs, community teams or emergency services where appropriate
Support care that reduces the risk of deterioration and avoidable hospital admissions
Clinical Governance & Compliance
Ensure compliance with NMC Code of Conduct, CQC regulations and internal policies
Maintain accurate, contemporaneous clinical records
Contribute to audits, incident reporting and clinical risk assessments
Support the Registered Manager during CQC inspections, providing clinical evidence and assurance
Contribute to continuous quality improvement
Medication Management
Oversee safe medication practices in line with legislation and policy
Review MAR charts, medication incidents and errors
Liaise with GPs, pharmacists and other healthcare professionals
Provide guidance and training to staff involved in medication support
Safeguarding
Identify, assess and escalate safeguarding concerns
Ensure accurate documentation and reporting in line with local procedures
Work collaboratively with safeguarding teams and senior management
Promote dignity, consent, capacity and best-interest decision-making
Leadership & Training
Provide clinical supervision and competency assessments
Support the training and development of staff delivering delegated nursing tasks
Contribute to inductions, refresher training and clinical skills development
Act as a positive clinical role model
Multi-Disciplinary Working
Communicate effectively with Service Users, families and healthcare professionals
Attend reviews, MDT meetings and case conferences as required
Support coordinated, integrated care across health and social care
Skills & Experience
Essential
Registered Nurse (Adult) with current NMC registration
Experience in community nursing, complex care or long-term conditions
Strong understanding of clinical governance and safeguarding
Ability to work autonomously in the community
Excellent communication and documentation skills
Desirable
Experience supporting CQC inspections or audits
Experience supervising or assessing staff competencies
