Designed to bridge the gap between hospital discharge, supported housing, and independent living, our service focuses on stability, progression, and long-term outcomes.

What is Transitional Social Care?

Our transitional social care services support individuals who may be:

Moving from hospital discharge into the community

Requiring short-term or long-term residential support

Living with mental health needs, disabilities, or age-related conditions

We focus on building life skills, confidence, and routine, helping individuals move forward in a safe and supported way.

We currently operate five residential care properties across Greater Manchester, each designed to provide a safe, supportive, and structured living environment. All homes are staffed by trained, experienced carers and follow CQC-aligned care standards, ensuring high-quality support at all times.

Suitable for Able-Bodied Females

A smaller, more intimate residential setting that provides a calm and supportive environment. Ideal for individuals who prefer a quieter atmosphere while still benefiting from shared living.
