My Story

Katherine’s career with Westwood Homecare started in 2019.

Katherine plays a vital role in coordinating and overseeing the delivery of comprehensive care services to individuals in their homes. Her primary objective is to ensure that clients receive high-quality, personalised care that meets their physical, emotional, and social needs.

Katherine serves as the central point of contact for clients, families, care workers, and healthcare professionals involved in the client's care. She coordinates various services, including nursing care, personal care assistance, therapy services, medical equipment provision, and social support—ensuring that all aspects of the client's care are well-integrated and delivered seamlessly.

She regularly monitors the progress of clients and the quality of care provided by the care workers. Katherine conducts ongoing assessments, home visits, and reviews of care plans to ensure that clients' needs are being met and to identify any necessary changes or adjustments.

A strong advocate for her clients, Katherine ensures that their rights, preferences, and dignity are respected at all times. She provides emotional support and practical assistance to both clients and their families, helping them navigate the complexities of the healthcare system, access community resources, and address any concerns or challenges that may arise.

Katherine also ensures that Westwood Homecare’s services comply with regulatory requirements, industry standards, and best practices. She monitors adherence to care protocols, infection control measures, and documentation standards to maintain high-quality care and mitigate potential risks.

She facilitates smooth transitions of care for clients who may be moving between different care settings, such as hospitals, rehabilitation facilities, or nursing homes. Katherine collaborates with healthcare professionals to ensure continuity of care and supports the client’s transition back to their home environment.

Katherine and her team regularly evaluate the effectiveness of care plans and services, actively seeking feedback from clients, families, and staff members. She uses this feedback to identify areas for improvement and implement changes that enhance the quality and effectiveness of care delivery.

Overall, Katherine’s role is multifaceted, requiring strong clinical, organisational, communication, and advocacy skills to ensure the provision of person-centred care that promotes the health, safety, and independence of clients.
