My Story

Sarah’s career with Westwood Homecare started in 2021. Sarah plays a vital role in coordinating and overseeing the delivery of comprehensive care services to individuals in their homes. Her primary objective is to ensure that clients receive high-quality, personalised care that meets their physical, emotional, and social needs. Sarah serves as the central point of contact for clients, families, care workers, and healthcare professionals involved in the client's care. She coordinates various services, including nursing care, personal care assistance, therapy services, medical equipment provision, and social support, ensuring that all aspects of the client's care are well-integrated and delivered seamlessly. Sarah regularly monitors the progress of clients and the quality of care provided by the care workers. She conducts ongoing assessments, home visits, and reviews of care plans to ensure that the client's needs are being met and to identify any changes or adjustments needed in the care plan. Sarah advocates for the best interests of clients, ensuring that their rights, preferences, and dignity are respected at all times. She provides emotional support and practical assistance to clients and their families, helping them navigate the complexities of the healthcare system, access community resources, and address any concerns or challenges that arise.
